We had to fight to get the surgery to remove my uterine polyp covered. Since it was the fertility doctor that found it, the insurance company didn't initially cover it, even though the doctor had included notes as to why it needed to be done (and they weren't simply fertility reasons). Tests, procedures, treatments, etc. were all out of pocket for us.
